Community Nurses

District Nurses

Our District Nurses provide care to people who are too ill or infirm to come to the surgery. They offer the following services: wound management, administration of certain medications, blood taking, chronic disease monitoring for certain conditions, minor illness advice. They are frequently involved with patients recently discharged from hospital requiring follow up nursing care and patients suffering from chronic or terminal conditions. They can be contacted at the surgery on 01948-860038.

The nurses can obtain specialised equipment, including continence products, to help with daily living. They also undertake health home screening and health promotion to help you stay healthy with guidance on weight, smoking, alcohol and all aspects of keeping healthy throughout life.

After 4pm for urgent nursing problems Western Cheshire PCT offer a 24 hour Nursing service which can be contacted on 01244-349631.

Auxiliary Nurses

The Community Nurses have special training to carry out nursing procedures in the patient's home, with referrals coming from hospitals, GPs and other agencies.

Community Matron

Sister Garner has worked at the practice since 2003 as a District Nursing Sister. She currently works in a new clinical role as Community Matron across Malpas, Farndon and Tattenhall and is studying towards an MSc in Clinical Case Management.

The case management work of Community Matrons is central to the Government's policy for the management of people with long term conditions. Ruth will be working with patients who have intensive care needs to help ensure their care is properly co-ordinated and to help them make choices about their health care.
