Diabetes X-pert Course

This is a six week structured group education programme for people with Type II Diabetes, where the participants learn about management and treatment of diabetes and have the opportunity to explore and address problems/issues that they may have with their diabetes.

The programme has been shown to improve diabetes control, reduce diabetes tablets/insulin in some cases, increase self-management skills, improve lifestyle and quality of life.

The course is run over six weeks, one session a week lasting 2 ½ hours and is facilitated by Sister Gill Evans, one of our practice nurses. The aim of the course is to develop self empowerment in people with type II diabetes to enable them to develop knowledge, skills and confidence to make informed decisions regarding their lifestyle and diabetes self management.

Each week is broken down into three areas these are Information sharing, communication and networking, lifestyle experiment and goal setting.
The course covers:

Week 1 – What is diabetes
Week 2 – Weight management
Week 3 – Carbohydrate Awareness
Week 4 – Food Labelling possible supermarket tour
Week 5 – Complications
Week 6 - Consolidation Questions and evaluation
