The Choose & Book Service

The Doctor has referred you to the hospital. You will have discussed which hospital you could go to. We have arranged your hospital appointment using the new Choose & Book service.

What are the benefits of using the choose & book service?

You will have a choice of which hospital to attend and you will be able to choose the date and time of your appointment.

Why do I have to consent?

Your personal details have been sent to the hospital electronically, for which you have consented.

What information do I need?

All the information you need is listed in an appointment request letter given to you when your referral is initiated at the surgery. This letter gives you your appointment reference number and a list of hospitals and clinics available. With this information, you can telephone the hospital or clinic of your choice, within 7 working days unless advised otherwise, and book your appointment using your individual Booking Number and Password. If you do not ring, you will be telephoned to remind you. You also receive a password for security. This is used to make sure that only you, or someone you have chosen to act on your behalf, can make or change your booking.

Do not keep your password and appointment reference number together, and ensure that both are kept in places where others cannot see them.

How do I book my appointment?

To book your appointment please call the hospital booking staff at the hospital or clinic you want to use. The telephone number is on your appointment request letter. If you want further information about making your appointment or about the hospitals listed on your appointment request letter please call the Choose and Book Appointments Line on 0845 60 88888.

How do I change my appointment?

If you want to change the date or time of your appointment, please phone your chosen hospital on the same telephone number which you used before and they will change your booking.

You will need your appointment reference number, password and year of birth. If someone else booked your appointment for you, the same steps outlined above will apply.
