Sick Notes
Please make a routine doctor's appointment if you are going to be off sick for more than 7 days (5 working days) and need an official sick note. Employers should not need an official sick note for 7 days or less, a self certificate should suffice. If your employer insists on a note before then or if you need a note for other purposes you will need an appointment for a private note for which you will be charged. Forms and Certificates
There is normally a charge for completion of forms - fees
If the doctor has seen you about the problem he may be able to complete the following forms without seeing you.
- Private treatment claim form
- Private Insurance application forms
- Private sickness insurance claim
- Holiday cancellation form (an official form from the travel/insurance company is normally required for this)
- Passport Application (check first doctor able and willing to do this)
Please complete your part of the forms before submitting and give details you think the doctor may need to complete his side e.g. the dates you were off work; when you booked the holiday.
Please bring the forms to reception or post them to us. Make it clear whether you are collecting the forms or are enclosing a SAE for return to you or to a third party. Allow 5 days for completion. It is a good idea to ring to confirm everything is OK and to check when the forms are likely to be ready for collection.
Make appointment
If the doctor has not seen you about the illness relating to the above forms you will need to make an appointment. Bring the forms with you and be prepared to leave the forms for collection at a later time. The following normally need an ordinary doctor's appointment.
- Private sick notes
- School attendance / missed exams
- Fitness to travel
- Any forms relating to referral on to another therapist/service
Medical Examinations
The following need a special longer medical appointment. Advise the receptionist the nature of the medical so an appropriate appointment can be made and make sure that either the surgery has the necessary forms or you bring them along with you. For unusual medicals not listed here bring the forms to reception so we can judge what is involved.
- Life insurance
- Heavy Goods Vehicle (HGV) or Passenger Service Vehicle (PSV)
- Diving, Boxing and other sports
- Adoption
- Armed Forces application
Be prepared to produce a urine sample at the surgery, so hang on before yoru come! For life insurance you may be asked to give a saliva or blood sample for HIV testing or to test for tobacco use (Cotinine test).
Charges
Some reports e.g. Life Insurance, are usually paid for by the companies requesting them but for all the others there will be a charge.
